NOTICE TO BIOLOGICAL FATHER
PURSUANT TO OFFICIAL CODE OF GEORGIA SECTION 19-8-12(b)
This notice is given to you as the biological father regarding the adoption proceedings which have been filed and are attached hereto.
This notice is given to you pursuant to O.C.G.A. 19-8-12(b) regarding the termination of your parental rights in conjunction with the pending adoption which is attached hereto.
As the biological father, who is not the legal father, you will lose all rights to the minor Child and the Court will not send you further notice nor will you be entitled to the adoption of the minor Child unless, within 30 days of receiving of this notice, you do the following:
a) File a Petition to legitimate the minor Child pursuant to O.C.G.A. 19-7-22; and
b) You give notice of the filing of the petition to legitimate to the Court in which the adoption is pending, and to the undersigned person who provided this notice to you.
Failure to act as above will result in termination of all of your parental rights.
